South East London Integrated Care System (SEL ICS) is supporting an important programme, connecting care homes to the London Care Record, enabling staff can to provide residents with efficient and timely care.
The London Care Record provides instant access to key information about their residents, including hospital discharge summaries, their medications, previous medical history or plans for their care.
Not having access to this can make it harder for care home staff to provide the best care for their residents, often involving lots of calls to GP surgeries, hospitals or others trying to get the information they need. It can also lead to delays that impact on the care residents receive like administering medicines or discharges from hospital.
To improve this, a pilot programme began in late 2022 connecting the first 28 care homes in the Capital to the London Care Record.
The pilot programme was an overwhelming success helping to improve the care residents received while also saving care home, GP surgery and hospital teams valuable time. In summer 2023 the OneLondon Board took the decision to roll out access to the London Care Record to more of the Capital’s 1,500 care homes.
This is a major project so it will take some time and we will roll it out on a phased basis over the next couple of years beginning with the Oaks and Baring Lodge Care Homes in South East London in May 2024.
