Support available for interface projects bridging primary and secondary care
Unboxed is working on behalf of medical directors in south east London to support projects bridging primary care and secondary care.
The ICB has provided funding for Unboxed to identify and support three projects from across the integrated care system that are fostering smoother working between the sectors.
They are looking for both projects that are being planned or are currently underway, as well as those that are finishing or have recently finished. The scheme will not provide financial support to projects but will promote new thinking about service design and encourage engagement, collaboration, impact and learning.
