Lead for Frailty

My journey

My role is broadly to support the commissioning team, adult social care team and wider local health and social care system to understand and meet the needs of frail older people living in Bexley. This takes many forms, from contributing to reviews of local contracting arrangements for primary care provision to care home residents to providing practical support and education to care staff. The role has developed enormously during the pandemic, and I am really proud of the successes we have had as a cross-organisational team in supporting care home residents and staff in particular.
Current and planned work for this year includes:

• Participation in Home First commissioning work, including the development of virtual wards
• Education & empowerment programme for care staff with topics such as delirium and UTI
• Participation in prevention and health promotion initiatives such as development of the frailty strategy and a deprescribing project.
