Our work is focused on working with a wide range of stakeholders across south east London to deliver end-to-end pathway transformation. This includes:

  • engaging with those with lived experience to understand their experiences and how services can be improved;
  • developing of self-management resources to empower patients to manage their condition;
  • improving the resources available to GPs and other primary care colleagues so they are able to support patients in primary care (e.g. via advice and guidance);
  • developing and enhancing community services to reduce pressure on hospital services and deliver care closer to home; and
  • improving hospital pathways to ensure every appointment is value adding.

Recent achievements

South East London Waiting Times Website

We recently launched the South East London Waiting Times Website which allows primary care colleagues to access up-to-date information on waiting times for first outpatient appointments across the hospital sites in south east London. The website also incudes information on the alternative services available in each borough to remind referrers of what community provision or advice and guidance support is available. A patient view is currently in development and will be available in the coming months.

Community Services

Planned Care Community Services can reduce pressure on hospital services and ensure patients receive care closer to home. A key focus in recent years, has been to ensure that there is consistent provision across all six boroughs in south east London and we now have consistent community services in dermatology, ophthalmology and ENT across the ICS. We are the only ICS in London to do so. These services utilise a different workforce (e.g. GP with Extended Roles, optometrists), allowing us to increase overall capacity and reduce waiting times.
