One Bromley’s Adult Hospital at Home service has been awarded the prestigious Best in Primary Care and Diagnostics title at the LaingBuisson Awards 2024 for its entry, ‘Transforming Community Healthcare: Innovative Patient-Centered Care with Hospital at Home.’
The Hospital at Home service provides hospital-level care to patients in their own homes, ensuring they receive the treatment they need without requiring a hospital stay. Developed in 2022, the initiative is a collaboration between One Bromley partners, including Bromley Healthcare, the Princess Royal University Hospital (PRUH), Bromley GP Alliance, the South East London Integrated Care Board, and St Christopher’s Hospice. This collaborative model aims to address critical healthcare challenges, such as hospital bed shortages and the negative impact of extended hospital stays on vulnerable adults and older people.
The judges of the LaingBuisson Awards, which took place on Thursday 21st November, praised the service: “This service embodies the future of medicine by prioritising care before hospitalisation, utilising a brilliant multidisciplinary team approach. Its patient-centric focus, which includes involving patients in service design and addressing their concerns with innovative technology, is truly impressive.”
The Bromley Adult Hospital at Home service takes an innovative patient-centred approach, integrating patients’ needs and preferences into every aspect of its care model. It was developed through extensive consultation with patients and carers, using workshops and direct input to design pathways that best serve the community. This includes care for conditions such as frailty, palliative needs, and respiratory issues – all areas that were identified as previously underserved.
The service uses advanced technology, including remote monitoring and point-of-care diagnostics, to deliver care that is both efficient and proactive. These innovations have led to a significant reduction in hospital admissions, improved patient outcomes, and an increase in patient capacity. In 2023 alone, the service doubled its capacity, reduced its patient length of stay by 40%, and saw a 152% increase in monthly patient contacts.
